We're so happy to welcome a new 3D Fat Loss provider to the BodySite patient portal. Use the form below to enroll the provider. Our new HIPAA-compliant account creation rules require that we allow the provider (not us) to create or have access to the provider's account password.
So . . . after the form is filled out, the provider will receive an email confirming their enrollment. When they click on their activation link, they will be prompted to enter their company name, which will generate a matching subdomain and then they can create a unique password. :)
